If you are certifying your own employees, use the wallet cards as a proof completion of forklift training.

  • Forklift certification cards includes blank space to be filled in by the Manager/Supervisor with employee name, issue date and expiration date.
  • Verify Forklift Operators completed their Forklift Certification
  • Issue cards to operators according to OSHA Forklift Safety rules
  • Operators will carry wallet cards while operating forklift equipment
  • Wallet cards are printed on gloss for pen / marker writing
